CFC/UNITED WAY #8164 |
The Case Management Unit provides a range of services to deaf, mentally ill adults. Case managers in the unit work as a team, using a holistic approach. Staff work with each member with mental illness to ensure that their needs for living in the community are met. This includes arranging medical appointments, making family contacts, and tracking progress on goals. Members are actively involved in every step of their individual plans for success. The Case Management Unit is the crucial link joining members to their community.
Funding: Case Management Unit funding comes from the Commission on Mental Health Services. The contract administrator is St. Elizabeths Hospital.
Hours: 9 a.m. to 5 p.m., Monday through Friday.
